The AI Care Standard for Patient Communication establishes a modern, practical framework for the responsible use of artificial intelligence in patient-facing communication. 

As AI becomes increasingly embedded in healthcare workflows, this standard defines clear expectations to ensure AI-generated communication is accurate, safe, transparent, and aligned with clinical care teams.

Existing AI frameworks focus on technical performance and backend risk, not on the safety, clarity, and appropriateness of patient communication. As a result, AI can confuse patients, overstep clinical boundaries, undermine trust, and introduce real safety and liability risk. 

Healthcare needs a clear, defensible standard that defines how AI should behave when communication itself becomes care.

  • AI that mediates, generates, or meaningfully shapes provider communication with patients

This clear scope is essential to managing adoption risk, liability exposure, and patient safety.

This framework is designed to complement—not replace—clinical judgment, regulatory requirements, or established professional standards of care. It applies only to AI systems that communicate directly with patients or meaningfully shape provider-to-patient communication. 

AI that is used solely for internal analytics, administrative automation, operational decision support, or other backend clinical functions is intentionally out of scope.

The methodology used in creating the AI Care Standard was designed to ensure the Standard reflects real-world conditions and tradeoffs, not abstract ideals, and to give organizations confidence in using it to guide decision-making.

The AI Care Standard was informed by:

Safe and Clinically Accurate Communication

Patient-facing AI must communicate in ways that optimize health outcomes, prevent harm, and remain grounded in clinical accuracy.

AI systems should never fabricate or infer diagnoses, treatment plans, or results. They must acknowledge uncertainty, gaps, or contradictions in the medical record and surface potential documentation errors safely. All information provided must align with current, evidence-based medical guidance and rely on clinically credible sources and representative training data.

Robust testing—including research validation and shadow-mode deployment—must occur before patient use, with ongoing auditing to detect model drift. AI should never direct patients to take clinical action without explicit authorization from the care team.

Situationally Appropriate Responses

AI systems must respond appropriately to the psychological, emotional, and situational context of each interaction.

This includes detecting medical urgency, emotional distress, or social risk and escalating appropriately. Sensitive clinical contexts—such as cancer diagnoses, pregnancy loss, trauma, or genetic conditions—must be handled with heightened care. AI should recognize abnormal conversational patterns that may indicate impaired judgment and redirect high-risk or harmful intent to emergency or human support resources.

Tone, language, and content should adapt to a patient’s mental, emotional, and functional state, with validation across diverse patient populations.

Clear, Closed-Loop Communication

Effective patient communication requires confirmation of understanding.

AI should assess whether patients understand key information and adapt when confusion is detected. Communication loops must be closed for medications, follow-up instructions, and clinical red flags. Plain language should be used consistently, tailored to the patient’s cognitive level and health literacy, ensuring clarity without oversimplification.

Trust and Patient-Specific Accommodation

AI systems must adapt to individual patient needs to build trust and ensure accessibility.

This includes automatically adjusting to language preferences, communication modalities, and accessibility requirements such as large text, screen-reader compatibility, or voice narration. Reading level should be inferred and adjusted appropriately, with default use of clear, plain language. Where relevant, AI should remain sensitive to cost considerations and social determinants of health.

Patient Autonomy and Empowerment

AI should empower patients to understand their health while respecting clinical boundaries.

Patient curiosity and the right to understand care options should be validated through balanced, evidence-based explanations. AI must avoid biasing patient decisions and rely only on high-quality clinical literature. Patients should be consistently reminded that final decisions must be discussed with a qualified clinician, with transparent provenance logs supporting neutrality and evidence sourcing.

Disclosure of Identity and Training Limitations

Transparency is essential for ethical AI use.

Patients must always know when information is generated by AI rather than a human. Patient-facing tools should clearly introduce themselves as AI and consistently label AI-generated outputs within the user interface. Where ethically appropriate, alternatives to AI interactions should be available. AI systems must also disclose clinically relevant training limitations, such as populations or conditions not represented in training data.

Truth and Evidence-Based Information

All AI-generated claims must be verifiable and traceable.

Information provided to patients should link directly to clinical notes, primary data, or peer-reviewed medical and scientific literature. Systems should maintain metadata and lineage—including authorship, timestamps, and content type—and ensure transparency of reasoning. Source lists and data lineage should be available to administrators and, when appropriate, to patients.

Optimization of Care Team Workflow

Patient-facing AI should enhance—not burden—clinical workflows.

AI systems must reduce clinician workload and administrative friction while supporting the provider–patient relationship through aligned, respectful messaging. Sensitive issues should be addressed without unnecessary escalation or erosion of trust. Patient frustration should be acknowledged constructively, preserving human capacity for higher-value care.

Acknowledgement of Limits, Confidence Levels, and Reproducibility

AI must recognize and communicate its limitations.

Clinical information should only be communicated when supported by direct evidence in the patient’s health record. Strict minimum data thresholds must be established, with AI using clear deferral language—such as acknowledging uncertainty—when information is missing or contradictory. Responses should be consistent in the same context and defer to human clinicians whenever limits are reached.

Continuous Oversight and Improvement

Responsible AI requires ongoing governance.

AI systems must be continuously monitored through automated and human oversight to detect performance drift. Models should be adjusted or rolled back promptly if accuracy, clarity, or tone deteriorates. Oversight programs should track multiple performance dimensions over time, including before and after model updates, vendor changes, or core technology shifts.
